To register your child you need the following:

1. Proof of Residency:
• Parent/legal guardian information including address, phone number, and date of birth.  Parent/legal guardian must be present to enroll the child.
• If the child is living with a  person who is not the parent/legal guardian, you must go to the Student Transfer Department to complete a “Determination of Residence of a Minor”.
• Photocopy of picture ID, which must match residence address.
• Copy of current lease agreement.
• Letter from Apartment Complex on official apartment letterhead verifying current lease agreement.  The letter must have the child and parent/legalguardian’s name as living in the apartment listed in the lease agreement.
• Copy of current electric, water, or gas bill.

• Current immunization records from doctor or clinic
4. Birth Certificate, baptismal certificate, hospital certificate, or passport.
5. Social Security Card, if available
6. Test Results, if available:
• TAKS test results
• Stanford test results
7. ARD Papers, if applicable
